Note that these requirements apply only to associations you have with organizations outside of JHU that are not handled through the university or sponsored projects (i.e., JHURA). Funds paid to your department, lab or to you as salary do not need to be disclosed.
You must disclose the following types of outside activities provided to ANY entity, whether non-profit or for-profit, regardless of whether you are paid for this activity:

Receipt of income from licenses, whether equity, royalty or other
Entitlement to equity and/or royalty
Some exceptions:
Service on Advisory Committees, Boards and Review Panels; Speaking, Teaching, Lecturing when provided to a U.S. state, federal or local government agency, teaching hospital, medical center or institution of higher education or related research institute.
According to University policy, disclosures should be made before engaging in outside activities or acquiring financial interests. In addition, once a year you will be asked to certify that your disclosures are up to date.
